OVERVIEW

The Patriot is a progressive, nonprofit Nursing and Rehabilitation Facility operating 100 fully licensed nursing beds and 76 Personal Care/Assisted Living accommodations. All 100 nursing beds are Medicare and Medicaid certified. We accept a variety of insurances; we also accept private pay status. To complement our adult services, an in-house child Daycare with "The Patriot Kids" offers service to employees and the general public.

Various levels of healthcare, individualized to need, are provided to age ranges from the young adult through the senior citizen. An independent practitioner serves as Medical Director, lending her expertise to our mission.

Our aggressive, fully staffed Rehabilitation Program operates six days per week. Therapy services are offered not only to those recuperating from procedures such as hip and knee surgery, but also to those needing to regain strength and endurance from diagnosis such as pneumonia, heart attack and stroke.

A Palliative Care Program utilizes licensed Hospice Services as needed; our Serenity Suite offers emotional and physical support to the resident and family during the last days of life. Monitoring and intervention by licensed staff and support personnel is provided around the clock.

Intergenerational programming, the connection between residents and The Patriot Kids, is an eagerly anticipated event occurring throughout the week.

Our Personal Care level offers a supervised, homelike setting that can accommodate the highly independent individual through those requiring an increased level of assistance. Two Independent Living Suites, complete with a kitchenette and private bath, have recently been completed.

Respite Care is available within the Nursing or Personal Care component. This allows the much needed break for a caregiver, with their loved one being cared for in an environment that is responsible, comfortable and conveniently located.
